INITIAL CONSULT
During your first visit, Dr. Kominos will obtain a detailed medical history. Prior to the visit you will complete an intake form describing your reason for the visit, any symptoms or concerns you have, your family history, and details of your lifestyle. This form will help guide your consultation. After obtaining your history, Dr. Kominos will examine you and may perform an EKG. Any findings will be discussed with you. Finally, we will outline a treatment plan. This may include an exercise prescription, drug therapy, referrals for additional testing, or other treatment. When possible, natural remedies will be employed. 
 
FOLLOW UP VISITS
At your follow up, results of tests are discussed, effects of any prescribed treatment are analyzed, a physical exam is repeated, and a new treatment plan may be outlined. 

GENOME TESTING AND COUNSELING
Genes are not destiny! But they provide information about risks for disease. These risks can be minimized since genes can be "turned on" or "off" with appropriate measures. After a complete consultation and discussion of benefits and limitations of genetic testng, advanced genome testing can be ordered to determine unique risks for chornic diseases such as Alzheimer's disease, heart disease, macular degneration and more. Based on results, a personalized plan will be created to minimize risk. This office uses the most advanced genetic testing, IntellxxDNA. 
 
EKG
Using painless, disposable, one-time use electrodes attached the skin in specific areas, an EKG gives information about the heart's health and rythm.
  
REFERRALS
Dr. Kominos has aligned herself with top integrative practitioners in nutrition, accupuncture, and mind-body therapies. She is also affiliated with expert conventional cardiologists who specialize in nuclear, interventional, and vascular cardiology. 

INSURANCE:
This practice has opted out of insurance plans. Payment is required at the time of your visit by cash or check. Credit cards are not accepted. After your payment is made, if you do not have Medicare as your primary insurance, you will be given a receipt that you can submit to your insurance carrier (see Medicare patients below). This office cannot submit bills to your insurance carrier.

Medicare patients:
If you have Medicare as your primary insurance, you, or this office, legally cannot submit any receipts for reimbursement from Medicare. This is not our policy but the law. Medicare also requires that you sign a standard contract with this practice agreeing that you will not submit for reimbursement.  If you have secondary insurance, you will be provided with appropriate form that you may be able to submit to the secondary carrier.
